To refer a patient to Harbor Physical Therapy please fill out

Once the patient has been evaluated by a physical therapist,
you should expect a comprehensive evaluation report
mailed to your office within the week. When applicable,
you can also expect regular updated progress reports
to be faxed to your office.

​

Our office will verify the patient's physical therapy
insurance benefits. If you are referring a patient claiming
workers' compensation, we will contact the adjuster
and request physical therapy authorization. In the case
of referring a patient claiming workers' compensation,
it is appreciated if the adjuster's name, phone number,
and all other related information be faxed to our office
along with the physical therapy referral.
